Understand Your Risks and Stay on Track
Your nurse will take the time to understand your heart health, daily routine and any factors that may be affecting your wellbeing or ability to work safely. This may include reviewing your blood pressure, cholesterol and glucose levels, along with physical activity, smoking or vaping, nutrition, sleep, stress and other relevant lifestyle factors.
Together, you will identify realistic goals and build a practical plan that fits your health needs and everyday life. Your nurse can support you to make gradual changes, such as becoming more active, managing stress, improving sleep, making heart-healthy food choices or reducing smoking and vaping. These changes can help support your long-term health, concentration, energy and safe participation at work.
Your coach will check in on your progress, help you work through barriers and adjust the plan as your needs change. The service supports health education and sustainable behaviour change. It does not replace medical diagnosis or treatment, and your nurse may recommend speaking with your GP or another health professional where appropriate.
